Passport Forms

IMPORTANT: The DS Form 11 must be signed in front of a Consular Officer at the US Embassy. In order to sign the DS Form 11 in front of a Consular Officer, one must make an appointment to visit the US Embassy.

This form is for applicants who:

  • Cannot present their last passport (lost or stolen);
  • Have never been issued a US passport;
  • Hold a passport issued more than fifteen years ago;
  • Hold a five year passport.

Special instructions for passports for children under 16.

If one of the above apply to you, please come personally to the Embassy with one or more of the following documents:

  • Original U.S. birth certificate or original U.S. Consular Report of Birth Abroad;
  • One of your previous U.S. Passports;
  • Certificate of Naturalization;
  • Current Photo ID.

You will also need to bring:

  • Photograph:
    • One 2″ x 2″ (5 cm x 5 cm) color photograph taken within the past 6 months; full face, frontal view against a plain white or off-white background; size of head should be between 1 inch and 1 3/8 inches (2.5 cm and 3.5 cm) from the bottom of the chin to the top of the head. Danish passport size photos are not acceptable.
  • Fee:
    • Over 16 years old: US$ 135.00/DKr. 945,00.
    • Under 16 years old: US$ 105.00/DKr. 735,00
    • Fee can be paid by cash, credit card or Danish kroner checks.  Checks should be made out to “U.S. Embassy”.
  • If you wish your passport to be sent to you please provide us with a self addressed stamped envelope  with 98 Dkr in stamps, or 107 DKr if more than one passport.
  • If you live in Sweden, please provide with a self addressed stamped envelope with 104 Dkr in stamps or Dkr 114 if more than one passport.
  • You will need to bring photo copies of all original documents presented including the passport to be renewed.

The Passport and Citizenship Unit is open to the public between 9:00 and 12:00 noon daily, except Thursdays, American and Danish holidays.

To complete Form DS-11

IMPORTANT: The DS FORM 82 does not require a visit to the Embassy. Please see the instructions above if you prefer to mail your application to the Embassy for processing. However, if you wish to submit your application to the Embassy in person, you must make an appointment.

You may use this form if:

  • You can submit your most recent passport;
  • You hold a 10 year passport which was issued within the past 15 years; and
  • You are not requesting a change in the passport data, other than a name change due to marriage or divorce or court order for which you can submit official documentation (marriage certificate, divorce decree, or court order) to support name change.

Please complete the entire form by typing or printing clearly in black ink. Be sure to sign and date the form where stated.

Submit the following items along with the completed form:

  • Your passport
  • Photograph: One 2″ x 2″ (5 cm x 5 cm) color photograph taken within the past 6 months; full face, frontal view against a plain white or off-white background; size of head should be between 1 inch and 1 3/8 inches (2.5 cm and 3.5 cm) from the bottom of the chin to the top of the head.   • Fee: US$ 110.00/DKr. 770,00. For payment made at the Embassy, we accept cash, credit card or Danish kroner checks.  For applications submitted by mail, only Danish kroner checks are accepted. Checks should be made out to “U.S. Embassy”.
  • If you wish your passport to be sent to you please provide us with a self addressed stamped envelope  with 98 Dkr in stamps.

All fees subject to change. Please allow three weeks for passport processing, as all passports are printed in the United States.

Applicants in Sweden should submit their application to the U.S. Embassy in Stockholm. Complete instructions for renewing a passport by mail in Sweden can be found at the U.S. Embassy Stockholm website
