APPLY FOR A PASSPORT AFTER A NAME CHANGE THROUGH MARRIAGE / DIVORCE OR ANY OTHER MEANS
If you have changed your name by MARRIAGE OR DIVORCE, you may apply for a new passport to be issued in your new name, using form DS-82 or DS-5504. If you have changed your name any other way, you must apply using form DS-11.
- If your current passport was issued less than one year ago and you consequently got married/divorced, you may apply for a passport in your new name by using Form DS-5504. There will be no charge for routine processing such an application. Please follow the instructions below on how to proceed.
- If your current passport was issued more than one year ago and you consequently got married/divorced you may apply for a passport in your new name using form DS-82.
If you have changed your name by ANY OTHER MEANS, you may apply for a new passport to be issued in your new name, using FORM DS-11.
STEPS TO FOLLOW:

Social Security Number Requirement
You must provide an answer to question no. 5, even if you do not have a Social Security Number or you do not remember it. Do NOT list your Danish CPR number.
For an applicant that does not have U.S. Social Security Number, you must submit a statement.
On your application form you must write the word ‘NONE‘ at item no. 5 on your DS-82 or DS-5504 form. Do NOT write zeros.
This statement will work only once. Upon receipt of your new passport you must apply for a Social Security Number asap. The application begins by contacting the Federal Benefits Unit at the U.S. Embassy in Oslo, Norway
Proof of U.S. Citizenship
Provide your most recently issued full validity (10-year) U.S. passport, with a photocopy of the biographical information page.
One 2"x2" (5x5cm) U.S. format color passport photo

Evidence of Name Change
Through marriage or divorce:
You will need to submit your original marriage or divorce certificate. Documents presented must be in English or be accompanied by a sworn translation into English. If your divorce decree does not state your maiden name, and you wish to have that name in your passport again, please also provide your birth certificate.
*Please note that if you have changed your name through marriage in Denmark, you don’t need to submit your Danish marriage certificate, but an Attestation of Name Change document, instead (see instructions below).
Through any other means:
Bring the original documentation, with photo copies, supporting your name change.
This can be a variety of instruments, e.g. an official name change certificate, a court order, or a deed poll.
If you have changed your name in Denmark, please include original Attestation of Name Change (Attestation af Navneændring) issued, signed and stamped by the local church office (sognekontor). Please note we will not accept a print out from your Nem-ID account.
Documents presented must be in English, or be accompanied by a sworn translation into English.
Aside from the original(s) provide a plain photocopy of your passport’s biographical information page and any supporting documents as applicable to your application. Copies of the forms are not required.
Passport Application Fee
Form DS-5504 is free of charge.
Form DS-82: If you are using form DS-82 and you mail in your application, pay the $110 passport application fee via the U.S. government’s secure payment site. You must print the electronic payment confirmation you receive via e-mail after successfully paying the fee and include it in the package you send to the embassy.
Form DS-11: You can pay the appropriate passport services application fee in DKK KR, in U.S. dollars, or by credit card. Note that we can only process credit card payments if the card holder is available to sign the payment authorization slip. When paying in cash, please try to bring exact change. *It’s important you include your name and address as sender on the top left corner of the envelope. Envelopes received without it, won’t be allowed into the compound due to security reasons.

You will received an email informing you the documents are ready for pickup. Please bring copy of this e-mail, photo Id and your previous passport if you did not turn it in when you applied for the passport. No appointment is necessary.
Pre-paid self-addressed envelope if you want your new citizenship documents mailed back to you. IMPORTANT MAILING INSTRUCTIONS: Please purchase a pre-paid mailing label on the Post-Nord website: https://www.postnord.dk/en. Please select “Rek & Value Traceable” and pay 96 DKK for mail within Denmark. For mail to Greenland, the Faroe Islands, and Sweden please select the appropriate country from the drop-down menu, then select “Rek & Value Traceable” and pay 132 DKK. Retain your tracking number. We are unable to mail to any other locations. Please do NOT use postage stamps on your envelope.
